The Health for Friends Clinic is located in the Community Health Building at 1318 Broadway (Broadway and Avenue M) in Lubbock. It is nurse-managed and provides free health screening, one-on-one support and lifestyle education to low-income adults with (or at risk for) chronic health issues such as diabetes, hypertension, high cholesterol, and morbid obesity. These patients have little access to health care because they don't have medical insurance and do not qualify for Medicaid or Medicare benefits. Referrals are made to physicians as necessary; personalized meal plans are developed; and individual consultations are provided concerning diabetes maintenance, high blood pressure maintenance, nutrition counseling, cholesterol management, and weight control. Recognizing that many of the potential clients have no means of transportation, the Health for Friends Clinic brings their services to various outreach locations including several locations in Levelland and Slaton on a rotating basis. Spanish is also spoken at the clinic, and resources are printed in both Spanish and English. During 2004, Minga and Tami covered 3,039 clinic visits and 5,745 patients were seen at the 12 outreach sites. Of the 143 patients seen 5 or more times in the clinic, 93% showed improvement -- a remarkable achievement!
